Smoking Cessation Discharge Advanced Service – Local Postcard

The UHB Tobacco Control Advisors are now operating across all UHB’s hospital sites and are having conversations with inpatients who are flagged as a “smoker” at admission. Many contractors have now signed up to this advanced service and receive referrals via their PharmOutcomes inbox. It is now becoming apparent that patients are forgetting about these conversations when they get home and so some are “lost to follow up”.

To mitigate this, the LPC and UHB have produced a postcard which will be given to the patient so they have a “memory-jogger” for when they get home.

A copy is attached so we can support more contractors to successfully help patients to quit smoking following their hospital stay.
